Small-Scale Multifunctional In-Home Care Facility Familiar House Appendix to the Statement of Important Matters
Small-scale multifunctional in-home care facilities (see Article 9 of the Operational Rules)
Please pay the service fee according to the fee schedule below.
The service fee varies depending on the level of nursing care required by the contracting party and the individual's percentage of responsibility. The service fee is the total of 10%~30% of the nursing care fee, plus the cost of housing, meals, and other usage fees.

■Service Fees

Fees are based on the publicly announced amount of nursing care fees and are inclusive (fixed amount) per month.
The service fee will be charged according to the user's level of care according to the fee schedule below.

Additional I Description
Initial addition: For a period of 30 days or less from the date of registration at a small-scale multifunctional in-home care facility per day, a co-payment is required for the additional amount as follows as an initial addition. The same applies when services are re-started after a hospitalization of more than 30 days.

Additional fee for strengthening service delivery system (I)
(1) Individual training plans have been prepared for all caregivers, and training has been or is scheduled to be provided.
(2) Meetings are held regularly for the purpose of communicating information and points to keep in mind concerning users and providing technical guidance to employees of small-scale multifunctional in-home care.
(iii) The ratio of care workers to the total number of employees excluding nurses or assistant nurses of said designated small-scale multifunctional in-home care business office is 70% or more.
70 or more. At least 25% of the total number of employees must be caregivers who have been with the company for at least 10 years.
(iv) The criteria for the calculation method of day-care expenses, etc. are not met.

Additional fee for strengthening service delivery system (II)
(1) Individual training plans have been prepared for all caregivers, and training has been or is scheduled to be provided.
(2) Meetings are held regularly for the purpose of communicating information and points to keep in mind concerning users and providing technical guidance to employees of small-scale multifunctional in-home care services.
(iii) The ratio of care workers to the total number of employees excluding nurses or associate nurses of said designated small-scale multifunctional long-term care business facility is 50% or more.
(4) Conform to the capacity and personnel standards specified in another public notice.

Additional fee for dementia (III)
Users with dementia who require nursing care because of symptoms or behaviors that may interfere with daily life (Dementia level of daily living independence III or higher)

Additional fee for dementia (IV)
Users whose level of need for nursing care is Care Level 2 or higher, and who have symptoms, behaviors, or communication difficulties that interfere with their daily lives and require the attention of those around them (Dementia Independence Level ll)

Additional charge for the assignment of nursing staff (I) At least one full-time nursing staff must be assigned to the facility.
Additional charge for nursing staffing (II) At least one full-time associate nurse must be assigned to the facility.

Additional payment for improvement of treatment of nursing care workers (II)
This calculation is available to business establishments that have notified the designating authority for the purpose of improving the treatment of nursing care workers that conform to the standards specified by the Minister of Health, Labour and Welfare.

Items related to direct services to users (1)
Of the total number of care workers, the ratio of those who have completed basic care shellfish training and first- or second-level home care worker training is at least 50%.
Items related to direct services to users (2)
Accepting users who require nursing care who fall under nursing care level 3 or higher and have independence in daily living level E for the elderly with dementia (this will be an addition to the eligible users).

Items related to contributions to the community, etc.
In order to establish a system in which local residents who are not registered residents can casually drop by the facility and also to promote interaction with local residents, the facility holds an event at least once a month in which local residents can participate, or participates in an event held in the community at least once a month.

<Support for short-term use and accommodation needs in emergency situations
When there is a vacancy in the accommodation room, the room may be used by non-registered guests under certain conditions, such as when there is a vacancy in the registered capacity and an emergency or unavoidable hot water situation arises.

Additional II Content Explanation
Additional fee for strengthening comprehensive management system (I)
Individual service plans are reviewed as needed based on changes in physical and mental conditions and the environment surrounding the family, and information is provided to hospitals and clinics on a daily basis. In addition, opportunities for participation in community activities are secured.
A system is in place to provide consultation to local residents and others.
Effective use of local resources to support users according to their conditions.

Additional fee for coordination for improvement of daily life functions I
A system that enables care support specialists to receive advice from physiotherapists, occupational therapists, speech therapists, and physicians at business facilities that provide in-home or daytime rehabilitation services or medical facilities that provide rehabilitation services (in principle, limited to those with less than 200 licensed beds) is established, and upon receiving such advice, care support specialists create (modify) small-scale multifunctional in-home care plans for the purpose of improving daily living functions. (3) The care support specialist shall create (modify) a small-scale multifunctional in-home care plan for the purpose of improving the daily life function.
In addition, physiotherapists, occupational therapists, speech-language pathologists, and physicians must regularly provide advice based on an understanding of the user's condition at the place where daytime rehabilitation, etc. is provided, or by using ICT-based videos, etc.

Additional fee for coordination for improvement of daily life functions II
A physical therapist, occupational therapist, speech therapist, and physician from an office that provides in-home or day rehabilitation services or a medical facility that provides rehabilitation services (in principle, limited to those with less than 200 licensed beds) shall visit the user's home and jointly conduct an assessment of physical functions (lifestyle function assessment). The care support specialist must create (or modify) a small-scale multifunctional in-home care plan for the purpose of improving daily living functions.

Additional fee for acceptance of users with juvenile dementia (the same applies to additional fee for acceptance of users with preventive juvenile dementia)
To assign an individual person in charge for each user with dementia under 65 years of age and provide services according to the characteristics and needs of the user.

The following services are charged at full cost to the subscriber.

d. Recreational activities, etc.
Subscribers may participate in recreational activities at their request.

(e) Delivery of copies
The subscriber may inspect the records of services provided at any time.
However, if photocopies are required, the cost will be borne by the applicant.
Fee: 10 yen per sheet

(f) Actual expenses necessary for daily living
The subscriber will be responsible for the cost of purchasing daily necessities and other expenses that are deemed appropriate to be borne by the subscriber.
